Oakridge Park is recruiting for two full time Care coordinators to work a hours per week.
Core hours:
1. Monday 11 till 19:00,
2. Tuesday 11:00 till 19:00
3. Wednesday 11:00 till 19:00
4. Thursday 11:00 till 19:00
5. Friday 11:00 till 19:00

Main duties of the job
The duties and responsibilities to be undertaken bymembers of the care coordinator team may include any or all of the items in thefollowing list.
6. Duties may be varied from time to time under thedirection of the senior receptionist/practice manager, dependent on current andevolving practice workload and staffing levels:
7. Opening up/locking-up of practice premises andmaintaining security in accordance with Practice protocols
8. Maintaining and monitoring the practiceappointments system
9. Book appointments for clinical staff in accordancewith Practice Policy
10. Take telephone and face to face requests forappointments and deal in a calm, courteous and helpful manner.
11. Processing and distributing incoming (and outgoing)mail
12. Taking messages and passing on information
13. Filing and retrieving paperwork
14. Processing repeat prescriptions in accordance withpractice guidelines
15. Computer data entry/data allocation and collation/clinical coding; processing and recording information in accordance withpractice procedures
16. Initiating contact with and responding to requestsfrom patients, other team member and associated healthcare agencies andproviders
17. Providing clerical assistance to practice staff asrequired from time to time, including word/data processing, filing,photocopying and scanning
18. Keeping the reception area, noticeboards andleaflet dispensers tidy and free from obstructions and clutter

The Practice provides GP services to the expanding areas of Northern Milton Keynes. There are now Four partners, and a large multi-disciplinary team of other professionals and support staff. All are committed to providing patients with a high quality and personal service.
The surgery
The building has been designed to provide a pleasant and welcoming environment and it is equipped to very high standards. There is a large free car park. Please park considerately to allow maximum use.
The area nearest the surgery entrance is reserved for disabled patients. For patients who are disabled there is easy access, and all consulting rooms are on the ground floor.

The purpose of the role is to:
19. Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
20. Undertake a variety of administrative duties to assist in the smooth running of the practice including the provision of secretarial and clerical support to clinical staff and other members of the practice team
21. To process all relevant prescriptions as authorised by GPs and prescribing nurses where appropriate, in accordance with the principles of good practice.
22. Answer the telephone promptly and book appointments according to the Practice Policy in a courteous and helpful manner.
