We are looking for a highly motivated nurse who is experienced in the care of people with chronic lung conditions. They will be responsible for the coordination of care of inpatients at Nottingham University Hospital who have been admitted with an exacerbation of COPD/ILD/bronchiectasis/asthma who are deemed as a respiratory readmission or at high risk of readmission.
They will work across respiratory medicine at Nottingham University Hospitals within the COPD nurse specialist team and will also work closely with other respiratory nurse specialist teams and members of the MDT. They will be expected to work independently with confidence as this forms a considerable part of the role.
As part of this role visits to the patients home may be required so a full driving license is essential. Attendance at community MDTs will also be expected.

1-identify inpatients within respiratory medicine that have COPD/asthma/ILD/bronchiectasis that are a readmission or are at risk of readmission
2- holistically assess identified patients to ascertain causes of/risks for readmission and formulate a comprehensive plan with them/carers/family/MDT members to optimise care and aid in future admission avoidance
3- refer appropriate patients to members of the MDT within primary and secondary care – to include specialist teams, Consultants, palliative care, OT, physiotherapy, dietician
4-liase with family/carers where appropriate and necessary
5-update appropriate people and systems with the plan of care
6-assist in post hospital discharge referrals to IROS
7- regularly meet IROS team to discuss patients meeting certain criteria
