Hours per week: 22.5-37.5
Salary: £30,000-£33,500 (WTE) - according to experience
ABOUT US
Mosaic Healthcare consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Brambly’s Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients spread between Basingstoke town centre and semi-rural areas such as Odiham.
We are committed to building a multi-disciplinary team and want to build on the pharmacy resource already within the PCN. All our ARRS staff are supported by an education lead to ensure we meet the aspirations of both the team member and the surgeries. The pharmacy team are also supported by a lead pharmacist.
JOB PURPOSE
The purpose of the role is to support improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines by switching medications according to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.
The post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. This will involve assisting the PCN in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives.
MINIMUM REQUIREMENTS
The post holder must:
1. be registered with the General Pharmaceutical Council (GPhC).
2. meet the specific qualification and training requirements as specified by the GPhC criteria to register as a Pharmacy Technician.
3. be enrolled in, undertaking or qualified from, an approved training pathway, such as the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
4. work under appropriate clinical supervision to ensure safe, effective and efficient use of medicines.
KEY DUTIES AND RESPONSIBILITIES
The duties listed are not exhaustive, which options are deployed will be decided by the individual practices within the PCN. Working within their competencies, the post holder will work under the supervision of the clinical pharmacist team to ensure safe and efficient use of medicines. All pharmacy technicians must have professional indemnity for their role.
Clinical Responsibilities:
1. undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
2. carry out medicines optimisation tasks including effective medicine administration (e.g., checking inhaler technique), supporting medication reviews, and medicines reconciliation.
3. support, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients.
4. provide specialist expertise, where competent, to address both the public health and social care needs of patients.
5. take a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients.
6. support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.
7. assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.
8. support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
Technical and Administrative Responsibilities:
1. work with the PCN multi-disciplinary team to ensure efficient medicines optimisation.
2. supervise practice reception teams in sorting and streaming general prescription requests.
3. provide leadership for medicines optimisation systems across PCNs.
4. provide training and support on the legal, safe and secure handling of medicines.
5. develop relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team.

COMMUNICATION AND WORKING RELATIONSHIPS
1. work collaboratively with their PCN multi-disciplinary team.
2. liaise with Lead Clinical Pharmacist to access professional support and training opportunities.
3. engage with clinical pharmacist team for peer support.
4. foster and maintain strong links with all services across the PCN.
PERSONAL & PROFESSIONAL DEVELOPMENT
1. work with your line manager to undertake continual personal and professional development.
2. attend mandatory training and fire lectures regularly.
3. keep up to date with and attend training on revisions to information systems.

SCHEDULE: Monday to Friday
WORK LOCATION: In person. The PCN is located across several sites within a small radius around Basingstoke and Odiham. This job role may involve travel across sites if required.
