Care Coordinator Duties and Responsibilities
As a Care Co-ordinator, you will be essential in managing and facilitating care services for clients, ensuring their needs are met with compassion and efficiency. Reporting to the Care Manager, your role involves coordinating care plans, liaising with healthcare professionals, and maintaining accurate records. Your core office experience will support administrative tasks, while your ability to communicate effectively will enhance client relationships. By leveraging your organizational skills, you will help create a seamless care experience, contributing to the overall mission of providing high-quality support to those in need.
Key Responsibilities:
* Ensuring that the staff delivers high-quality and personalized care to patients
* Managing budgets and ensuring the financial effectiveness of medical care settings
* Recruiting, training and supervising staff and communicating the needs of patients to Medical Staff
* Ensuring health and safety compliance
* Liaising with local community organisations and maintaining good relationships with stakeholders
* Ensuring that all activities such as personal care and medication administration meet the required standards
* Preparing reports related to the progress of patients
* Planning and utilising resources and staying updated with the clinical knowledge to ensure that patients receive the best care
Job Type: Full-time
Pay: £25,000.00-£28,000.00 per year

Experience:
* Domiciliary Care Coordinator: 1 year (required)
Licence/Certification:
* Driving Licence (preferred)
