An exciting opportunity has arisen to join the Hospice at Home team (H@H). Working alongside the multidisciplinary team, the H@H team provides high quality palliative and end of life care to patients wishing to remain in their own home.
We are inviting applications from candidates with a passion for palliative care and end of life care, looking for career development in this area. Candidates will need to work flexibly within the H@H service providing social, emotional, psychological, and wider well-being care to our patients and their families.

Person specification
Essential criteria
* Completion of application form relevant to post
* Evidence of understanding of the role of an HCA in a Hospice at Home team
* Evidence of team working
* Evidence of working autonomously
* NVQ level 2/3 or equivalent
Desirable criteria
* Hospice at Home HCA experience
* Communication skills training or evidence of courses/study

If this job requires you to have a DBS check to enable you to properly fulfil the requirements of the job, on appointment, you will be required to repay the cost of obtaining a DBS check and this amount will be reclaimed in instalments from your salary.
