We have an exciting opportunity for Nurses, Occupational Therapists or Social Workers to join Surrey & Borders NHS Foundation Trust (SABP) in the Reaching Out service as part of the Assertive Engagement Pathway. The Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network.
We are looking for clinicians who are creative, highly motivated, show initiative, and are passionate and committed to ensuring high quality services for people with mental health needs.
There are 2 full-time posts with SABP bases across the county.
Responsibilities:
1. To manage a caseload of young people referred to the service, working collaboratively with other services both internal and external, to provide consultations and mental health assessments to assist young people in overcoming the obstacles they face and achieving their potential.
2. To build a trusting relationship to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services. This role will involve outreach into the community to meet young people in their own or choice of environment.
3. To work in partnership with young people, families/carers, and other services regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs, and to co-produce solutions.
