This is an exciting opportunity to join our Maternity Stop Smoking Service here at Leeds Teaching Hospitals Trust, and be part of our commitment to reduce health inequalities across local communities and help pregnant patients and their families stop smoking.
This is a secondment opportunity to make a real difference to patients' lives by providing specialist support to help them stop smoking tobacco throughout their pregnancy and beyond. You will work independently with patients and their family members virtually, and face-to-face across community sites and during home visits, within the agreed Trust treatment pathways and policies. You will be based at our office at St James's Hospital and required to work from there on a daily basis.
We are looking to second an enthusiastic individual who is passionate about health promotion and supporting patients to become smoke-free. Previous experience in smoking cessation support or a health-care setting would be an advantage but is not essential, as training will be provided.
This is currently a fixed term secondment opportunity until 31st July 2025, however, there may be additional funding available to extend the contract until March 2026.

Main duties of the job
You will contribute to improving health and support pregnant patients and their families to stop smoking and develop healthier behaviours. Our Maternity Advisors work in a range of community settings and also visit patients in their own home, so having access to a vehicle and being able to travel across the city on a daily basis is essential.
You will be responsible for the timely delivery of personalised stop smoking support to patients and their families using interventions including: Nicotine Replacement Therapy (NRT), Vapes, Carbon Monoxide Monitoring and tailored behavioural support.
Job responsibilities
Principal Duties & Areas of Responsibility:
1. Acting as a key link between the Trust and community stop smoking providers to facilitate seamless and timely access for patients to continued stop smoking support.
2. Be the main point of contact for external agencies for patients experiencing delays in their transfer between providers of care.
3. Promptly alert the Team Leader of any significant delays or difficulties with a patient's access to continued care.
4. Participate in provider meetings to ensure a seamless transition of care takes place between providers.
5. Promote the health and wellbeing of people who use the service through offering specialist stop smoking advice, information and support.
6. Work with patients to change their behaviour to improve their health by stopping smoking (and, on occasions, support significant others e.g. partners/parents/family members who smoke).
7. Manage own caseload of patients and responsible for all associated tasks, including follow-up visits, and managing diary commitments.
Person Specification
Qualifications
* Educated to GCSE (or equivalent) grade C or above in English and Maths
* NVQ level 3 in Health care (or equivalent).
* Specific training in health promotion interventions
Skills and Behaviours
* Excellent communication skills (verbal and writing)
* IT skills, including data entry, accurate reporting of agreed care plans to support effective handover of care
* Good knowledge and understanding of evidence-based practice in stopping smoking, the consequences of smoking and the benefits of quitting.
* Ability to build and maintain good working relationships with professionals / individuals from different professional / cultural backgrounds
* Ability to work flexibly as part of a team
* Can work autonomously to achieve agreed objectives
Other Criteria
* Fully supportive of Trust's smoke-free policy acting as a smoke-free champion.
* Non-smoker due to the nature of the post
* Must be able to drive and have access to a car on a daily basis to be able to undertake community visits.
