Job summary The successful candidate will join our experienced multidisciplinary team of skilled and highly motivated staff in our 24-bedded inpatient community ward. We deliver quality services for patients requiring generic rehabilitation for older persons, deliver frailty, subacute and discharge to assess pathways. This opportunity will allow you to experience a holistic view of community services and gain knowledge of the full range of health and social services available for patients, both within hospital environment and other services in the community that can help the patient stay in their preferred discharge destinations safely.
