We are looking for a practice nurse to join our friendly, supportive and committed Practice Team. The successful individual will be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, and health prevention. A relocation package of £8,000 if moving from out of area and 5 weeks annual leave pro rata.
Main duties of the job
* Support patients with long term conditions.
* Perform treatment room procedures.
* Undertake Cervical Cytology.
* Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines, including Dopplers and removal of sutures.
* Carry out as required new Patient Medicals/Urinalysis.
* Offer lifestyle advice as appropriate.
* Undertake ECGs.
* Undertake Venepuncture.
* Support patients with Chronic disease management, undertaking annual reviews and follow-ups.
* Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
* Carry out Routine immunisations/Childhood immunisations.
* Provide Chaperoning and assisting patients where appropriate who are being examined by another clinician.
* Assisting GPs with minor surgery and other clinical procedures.
* Requesting pathology tests, for example urine culture, swabs.
* Following agreed clinical protocols with referral to senior nurses or GPs as appropriate.

* Maintaining and cleaning equipment used by the nurses and GPs.
* Ordering of stock.
* Ordering of vaccinations to maintain stock levels.
* Participation in administrative systems in the Practice.
* At all times there will be a need to maintain accurate records.
* Attend and participate in any Practice meetings when required.
* On occasion there may be a requirement to undertake home visits.
* Any other delegated duties appropriate to the post.
* Able to document Consultations according to NMC guidelines.

Qualifications
* Relevant nursing/health degree.
* Evidence of a commitment to continuing professional development.
Experience
* Knowledge of needs of patients with long-term conditions.
* Knowledge of health promotion strategies.
* Aware of accountability of own role and other roles in a nurse-led service.
* Aware of clinical governance issues in primary care.
* Ability to identify determinants on health in the local area.
* EMIS experience.
