An exciting vacancy has arisen for a part-time experienced practice nurse to work 3 days per week (Tues/Thurs/Fri), but with flexibility to cover annual leave when required.
Experience of working in general practice is essential.
Experience of using SystmOne is preferable but full training will be given.
The role is varied and covers all aspects of practice nursing.
Main duties of the job
The candidate should have core knowledge and skills in the management of long-term conditions i.e. Diabetes, Asthma, Dementia, etc. Childhood Immunisations, Travel Vaccinations, wound management, INR, ABPI, smears, and other enhanced services.
The candidate should have the ability to work in a team, have good IT skills, and strive to work for continuous improvement in their working role.
The candidate should be able to treat patients with dignity, respect, and compassion.

We are a very friendly rural practice with a list size of approx. 5300. Great Staughton is located five miles from St Neots with excellent transport links to the A1/A14. Our team comprises one GP Partner, two SGPs, one ANP, two PNs, one HCA, and a very supportive patient services team. We are also a dispensing practice. We were rated first in Cambs and Peterborough for patient experience in the 2023 GP Patient Survey.

Job responsibilities
To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice, the NMC code of conduct, and professional boundaries to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, and health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.
The following are the core responsibilities of this role:
1. To develop, implement and embed health promotion and well-being programmes
2. To implement and evaluate individual treatment plans for chronic disease patients
3. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health
4. To provide routine nursing care to patients as required in accordance with clinical based evidence, e.g., NICE
5. To provide wound care (ulcer/Doppler etc.) to patients
6. To request pathology services as necessary
7. To process pathology results as required
8. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required e.g., diabetes
9. To maintain accurate clinical records in conjunction with extant legislation
10. To ensure read codes are used effectively
11. To maintain chronic disease registers
12. To chaperone patients where necessary
13. To assist GPs with minor surgery when required ensuring consent received
14. To prioritise health issues and intervene appropriately
15. To support the team in dealing with clinical emergencies
16. To recognise, assess and refer patients presenting with mental health needs
17. To implement vaccination programmes for adults and children
18. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
19. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
20. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
21. To support the clinical team with all safeguarding matters in accordance with local and national policies
22. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
23. To deliver opportunistic health promotion where appropriate
24. To recognise and work within own competence and professional code of conduct as regulated by NMC
25. To participate in clinical audit and the implementation of changes to ways of working and/or clinical practice as a result of findings, recommendations and action plans
26. To maintain professional registration and practice in accordance with NMC requirements.
