Together Co is Brighton and Hove’s loneliness charity, creating connections to change lives for over 20 years. We do this through award-winning befriending, social prescribing and a range of volunteering programmes.
We’re excited to offer an opportunity for a Connection Hub Coordinator to join our team. The Connection Hub Coordinator serves as the initial point of contact for individuals and professionals contacting Together Co. This role ensures responsive, flexible support by directing individuals and referrers to appropriate interventions, resources, and support within Together Co or external organisations. Additionally, the role manages the Central Access Point for the UOK Brighton & Hove mental health service, guiding individuals and professionals toward local mental health resources. By streamlining access to services, the Connection Hub plays a crucial role in ensuring people across the city can quickly and easily find the support they need.
Key Responsibilities:
* Triage, person-centred support, and signposting.
This role is integral to ensuring responsive support, effective coordination, and innovation in service delivery at Together Co.
Hours: 25 hours per week, including a Wednesday (4 day minimum). Preferred hours can be discussed, however these will need to fit with the role requirements. The Connection Hub operates Monday to Friday, 9am to 5pm.
Salary: £27,198 per year full time equivalent (SP18) + 3% employer’s contribution to pension.
Status: This is a permanent role, reporting to the Senior Connection Hub Coordinator.
Minimum Requirements:
* Excellent interpersonal and communication skills.
* Strong organisational & decision-making skills with the ability to prioritise.
* Ability to work under own initiative in a busy and evolving service.
* Confidence in using a range of IT systems, including MS Office applications & CRM database systems.
* Knowledge of local resources, activities, groups and/or ability to actively research for places to refer.
