Job summary
The post holder will be based at Wexham Park Hospital, however may be required to support the service in Heatherwood and Frimley Park to cover annual leave and sickness. The postholder will be involved with the community and acute nursing services as part of an exciting opportunity to synergise the MS service. This role will also have specific responsibilities relating to co-ordintating the provision of disease modifying therapy drugs (DMT's) to patients with MS.
Main duties of the job
As a member of the MS multi-disciplinary team, the post holder will help facilitate the smooth running of the patient journey across all elements of the service and patient pathway by coordinating processes and monitoring the service provided to patients. The post holder will be responsible for the collection and monitoring of data pertaining to the service and contributing to service development.

Job responsibilities
Facilitate and co-ordinate the care pathway of patients on disease modifying
treatment (DMT) by:
Ensure relevant investigations ( MRI, CSF, blood results) are available to Consultant Neurologists and nurses in the DMT clinic to enable comprehensive assessment and initiation of most appropriate DMT.
Complete high-cost drug notification forms as required.
Establish and ensure the continuation of effective communication between the MDT team, external homecare delivery companies, injection support nurses, regional neurologists, regional MS nurses, GPs and the patient to enable seamless provision of DMTs.
Preparing the list of prescriptions sent in by homecare companies and obtaining recent blood results from GP surgeries if they are not on EPIC and uploading.
Monitoring patients attendance, following up as appropriate, particularly treatment-nave patients and those taking DMT and ensuring 6 monthly reviews with clinicians.
Telephoning patients with identified cognitive difficulties to remind them of OPAs thus reducing DNAs.
Printing and sending patient DMT letters
Ensuring patients attend relevant investigations as required to ensure safe monitoring of treatment.
Attendance at the DMT MDT to and follow up on tasks from this meeting
Booking patients into the MDT meeting
To support the ongoing development of research within the service and undertake appropriate tasks within research
Tasks and activities that support the MS team and enable effective delivery of care to MS and neurology patients commensurate with the position.

1. Experience of providing administrative support in a health or social care environment at Band 3-4 (or equivalent)
2. Experience of inputting and retrieving information from databases
3. Experiences with a variety of computer programmes
4. Experience of providing confidential administrative support to a complex and varied service
5. Experience of dealing with people in complex, challenging or distressing situations
Qualifications

6. GCSE or equivalent Grade C or above in English or Maths
