Job summary
Bridge and Monkseaton Medical Practice are looking to recruit a dynamic, compassionate and dedicated nurse with a passion for improving the quality of life for our elderly population.
You will be responsible for our aligned care home, Earsdon Grange, and other frail and elderly patients in our community who are either housebound or reside in other care homes and assisted living and are identified as having moderate to severe frailty, multiple co-morbidities, complex needs or who are identified as high intensity service users. You will work as part of the MDT at Bridge and Monkseaton Medical Practice. There will be considerable opportunity for innovation and job satisfaction.

You will have or be willing to train:-
A nursing or related degree
Advanced clinical assessmentskills accreditation
Extended prescribing skills
Advanced decision-making skills.
Proficientcommunication and consultation skills with patients, carers, care home staff,relatives, and other members of the primary care team.
Supportive skills across thepatient pathway - acute and community
Understandingthe main features of the Enhanced Care in Care home DES and helping thepractice adhere to this.
Deliveringboth reactive and proactive care to aligned care home patients.
About us
Bridge and Monkseaton Medical Practice is an enthusiastic, innovative practice that has 2 site locations. Bridge Medical is based in Shiremoor and Monkseaton Medical Centre is in Whitley Bay.
We are very focused on wellbeing and team development holding at least 4 Time in Sessions per year and we have clinical meetings most weeks. We have a strong focus on staff wellbeing and during the pandemic created a small garden and Zen room to take time out in. We also have regular and one off social and wellbeing events.
We are two practices with a patient population of 13,000, We have 3-partners, and we employ a management team, 9 salaried GPs, 2 Practice Nurse, 3 Health Care Assistants, 2 Paramedics, 1 Physicians Associate, 3 Pharmacists and a Pharmacy technician, 2 Mental Health and Well Being coordinators. We also work with the PCN and have ARRS roles in Physiotherapy, Social Prescriber, Frailty Nurses, paramedics for a home visiting service and administrators.
The Practices have excellent facilities at both sites for patients and staff. These include meeting rooms and communal kitchen area for lunch and coffee. The practices have long term lease purpose-built facilities.

Job responsibilities
Carry out weekly ward rounds atour aligned Care Home(s). This is currently Earsdon Grange.
CompleteEmergency Health Care Plans (EHCPs) for patients under your care and reviewthem when needed.
Complete DoNot Attempt Cardiopulmonary Resuscitation forms (DNACPRs) for patients underyour care and renew existing ones.
Manage the LTCs of patientsunder your care and complete LTC reviews.
Manage acute illness of patientsunder your care.
Undertake annual MH, LD andDementia reviews and create care plans of patients under your care.
To communicate with relativesabout patient care where appropriateincluding both acute and chronic management and also engage in discussionsabout future care planning.
To participate in palliative andEnd of Life care for patients under your care.
To communicate with secondaryand tertiary care and other MDT where appropriate, including referrals.
To participate in healthpromotion and disease prevention programmes immunisation programmes forpatients under your care.
To undertake training of carersand/or relatives to enable them to take and record basic observations so thatclinical decisions are well informed, and remote monitoring can be undertaken.
To attend meetings in practicewhere your contribution will be valuable Palliative care meetings, Audit meetings, SEA meetingsand PCN frailty meetings.
To participate in any researchprojects that the practice undertakes if it involves the patients who are underyour care.
Support the carers of thepatients under your care.
To support learners in the practice by teaching them more about your role and the Frailty service.

13. Understanding and knowledge of healthcare provision in GP surgeries, QOF and Enhanced Services
