Job overview
This 0.8 wte post is a replacement post for the Hospital at Home Service (virtual ward) which has been in place for almost 12 months and is proving to be of significant benefit to both patients and the local system.
Providing more acute care for patients safely and conveniently in their own homes is at the forefront of national frailty service design. The post holder will primarily be working within our developing ‘Hospital at Home’ service, providing acute medical care at home for older people living with frailty as an alternative to hospital admission. This post is in addition to existing medical cover provided by two consultants and will allow for more robust and consistent cover.
Main duties of the job
1. To work as part of the multi-disciplinary team in order to support and help to provide a comprehensive Geriatric Service within the Trust.
2. Maintenance of the highest clinical standards in the management of patients in their own place of residence.
3. Assisting with teaching, training and assessment of junior staff and medical students, nursing staff and other members of the multi-disciplinary team.
4. To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.
5. To have responsibility for ensuring active participation in continuing medical education (CME).

* MRCGP
* Diploma in Geriatric Medicine or equivalent experience
Experience
* Evidence of experience of working in a multidisciplinary team and effective team working skills.
* 12 years post medical degree experience including 6 years with specific experience in the area of elderly medicine.
* Evidence of leadership and management training.
You must have appropriate UK professional registration.
