Blandford Community Mental Health team seeks a Band 6 Community Mental Health Practitioner. As a Mental Health Practitioner, you will provide care delivery which considers people's safety, privacy, and dignity. You will provide interventions, assessments, and treatments, manage patients on a caseload, and plan, implement, and review health improvement programmes in a range of settings.
You will work within a supportive established integrated multi-disciplinary team. This team is made up of a Psychiatrist, Nurse prescriber, Mental Health Practitioner, Social Workers, OTs, Support Time Recovery Workers, and Psychologists.
The Community Mental Health Practitioner role is full-time Monday to Friday and based in Blandford, covering the surrounding villages. You will need to have a valid driving licence and use of a car as you will be required to assess and monitor patients in their own homes and within other community settings.
