Job summary Our City 1 Community Mental Health Team based at the City Anchorage on the Julian hospital site serving the Norwich City area is offering an exciting opportunity for a 8A Advanced Nurse Practitioner to join the team. We are looking for an experienced professional with exceptional organisation and communication skills to join our established high performing team. The role will incorporate oversite of all aspects of patient flow as well as providing care for presenting service users from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care and team audit. They will work along side the Community team manager, operational service manager and community matron demonstrating safe clinical decision-making and expert care, including assessment and diagnostic skills, for service users cared for in a mental health setting. The post holder will be able to demonstrate critical thinking in the clinical decision making process. They will work with the wider community team to meet the needs of the service users and provide clinical professional nurse leadership to the team. Main duties of the job The successful candidate will work as the senior clinician as part of our high performing community mental health team City 1, with oversite of the City 1 clinical team and wider community teams, assessing and meeting the needs of complex service users and helping with all aspects of patient flow from assessment to identifying treatment pathways to discharge. The post will be key to service improvement plans and will be responsible for quality audit processes and and helping further develop the team and service delivery. Duties can be broken down to cover clinical, Communication, administrative responsibilities, leadership, policy and service development, professional and educational and research and service evaluation.
