Clinical Nurse Specialist - Graft Versus Host Disease

Job overview
The successful candidate would work as part of a multi-disciplinary blood and bone marrow transplant team to provide a local and regional service to the transplant population with Acute/Chronic Graft Versus Host Disease (GVHD).
They would be responsible for co-ordination of day unit / clinic based post transplant care, completion of clinical skills tasks such as spirometry.
They will need to be the key worker for patients and their families, supporting decision making, communicating between various stakeholders and ensuring a smooth path whilst undergoing treatment for GVHD.
The post holder will be expected to collect data about their role, Quality of Life data such as PROMS and PREMS plus other outcome measures.
This post is fixed term/secondment for 12 months to meet the needs of the service.
Main duties of the job
* To deliver high quality and efficient haematology day centre / GVHD clinic and inpatient care for GVHD patients and their families
* To assess, plan, deliver and evaluate patient care using a holistic and individualized approach, based on the principles of best practice and clinical effectiveness
* To liaise closely with the AHP team and co-ordinate care as recommended
* To provide continued support to patients throughout their GVHD treatment journey
* To work with the ANP/Senior/Lead nurse and BMT Consultant lead to develop and update documentation against an agreed criterion maintaining a governance framework around practice.
* To provide patients with information, education and psychological support throughout the course of their disease and treatment
* The post holder will be required to work clinically in the outpatient setting
* The successful candidate will be required to enhance their clinical role by training in practical procedures such as cannulation/venepuncture/chemotherapy/care of central lines and factor concentrate administration.
* The applicant would be expected to cross cover other team CNSs and take part in the nurse led late effects clinic
* To maintain up to date nursing records and care plans
* Link to clinical trials team

Qualifications
* Registered Nurse
* Current NMC Registration
* Evidence of CPD
* Educated to Degree level
Experience
* Relevant post-registration experience in the same speciality and with related post-registration qualifications
* Previous experience of working in a variety of settings
Skills
* Evidence of up to date knowledge and expertise in relevant speciality
* Teaching and assessing skills
Personal Qualities
* Excellent communication and interpersonal skills
* Ability to work autonomously and as part of a team (MDT)
* Ability to prioritise and organise workload
* Able to work flexibly to meet needs of client group
* Good counselling skills
