Job Title: Care Coordinator Hours: 37.5 Location: Market Weighton Pay: £27,040 The business: Our client specialises in the provision of large care packages specifically designed for those who need a high level of care supervised by highly qualified nurses and support staff. This includes brain and spinal injury, life limiting conditions and enduring neurological conditions. A Learning Disabilities team provides all levels of care in this specialist area, including the provision of Independent Supported Living services, respite services and home support, specialising in support for those who have Autistic Spectrum Disorder and other complex conditions. Main duties of the job: 1. Allocate calls across all geographical areas to meet the needs of the organisation using the digital platforms available to ensure and reduce travel where possible. 2. Ensure service user notes are up-to-date and ensure staffing requirements are reported to management. 3. Ensure alerts are resolved in a timely manner and discrepancies are managed appropriately. 4. Ensure excellent communication with all staff, service users, colleagues and external professionals 5. Working alongside the assessment team to make simple referrals to District Nurses, GP’s and NHS Direct discussing this with the other members of the team and refer to 111 service where appropriate. 6. Refer all medication issues to the Registered Nurse for appropriate action 7. Refer all incident and complaint issues to the Registered Manager for appropriate action 8. Work with other members of the allocation team daily to prioritise tasks for the day and where required report issues to senior member of staff and/or request additional support if necessary. 9. Participate in the On Call Duty which includes evenings and weekends. 10. Provide emergency On Call and Care cover in times of illness and holidays. 11. Taking new Service Users referrals from social workers and private Service Users. 12. Answering and monitoring incoming telephone calls. 13. To be successful in applying for this role you will need: · Level 3 in health and social care (desirable) · 2 years’ experience working as a care coordinator · 3 satisfactory references · Enhanced DBS – adults & children
