This is an exciting opportunity for a motivated Physiotherapist to join a dynamic multidisciplinary community rehabilitation service in Southampton. The Community Independence Service is integrated with Community Nursing and forms the West Locality, delivering programmes of rehabilitation working in partnership with other social and healthcare providers.
The role will allow individuals to gain experience in falls assessment, education and prevention, complex community rehabilitation and goal focused rehab planning. Additionally, the role will involve provision of equipment and minor adaptations to support independence, providing advice on health and wellbeing and signposting to other community services. This will be supported by therapy staff, an advanced clinical practitioner and geriatricians.
The team promotes inter professional working which supports the single assessment process for patients. There is a robust supervision and appraisal framework within the team and opportunities for training and development. You will need effective interpersonal skills and to demonstrate skills to support a flexible and positive approach to problem solving.
