An exciting opportunity has arisen for enthusiastic individuals to join our Community Nursing Teams.

You will be part of a compassionate, forward thinking team working in one of the community nursing hubs. You will work closely with a team of registered community nurses, led by a District Nurse to deliver a wide range of care activities as detailed below.
Core service hours are between 8am and 6pm and you will be required to work weekends on a rota basis. As a flexible employer, we have a variety of shifts covering these hours. We believe in a positive work life balance and are open to discussing hours and patterns of work on an individual basis.

* Level 2 qualification in functional skills Maths and English or equivalent qualification
* Level 3 qualification in Care or equivalent relevant experience

Job Description Summary (Main duties and responsibilities)
Main duties of the job
* Providing clinical care delegated to you by a Registered Nurse, this includes, but is not limited to: management of wounds and leg ulcers, administration of medicines and catheter care.
* Providing personal care for patients.
* Working with the wider multidisciplinary team.
* Supporting patients to remain at home and avoid hospital admission.
* Promoting self-care and independence.
* To recognise and report changes in the patient’s condition, and any issues in respect of their health and management of care to the registered practitioner.
