Manage and co-ordinate clinical pharmacist led ADHD clinics across South West Herts, providing updates to the supervising GP in relation to the management of any pharmaceutical interventions and to adjust drug treatment appropriately with the support of senior team and working within agreed protocols and guidelines with the recommendation of continued prescribing shared to the child/ young persons GP within recommended guidelines of the NMC medicine management code of conduct and NICE guidance. Provide an integrated service working closely with wider team and liaising with colleagues in HPFT CAMHS and other services as required. As an autonomous practitioner, be responsible for monitoring patients with an ADHD diagnosis on medication through clinical pharmacist led clinics, communicating change to other members of the team, and giving guidance on pharmaceutical treatment of Children and Young People up to the age of 18. The post-holder will be able to take a holistic approach to care, considering a variety of aspects which may impact on the childs function and behaviour and ensuring to hear the child's voice within their review process. Act as a specialist resource within the service in respect of this client group working autonomously to support families and other health care professionals. To attend various multi-agency meetings, write reports and plan care, acting as the patient advocate when appropriate and to work in accordance with safeguarding childrens policies. Raise and discuss complex cases with supervising GP and ensure appropriate onward referral made. Promote patient and public involvement activities in the specialist area, by involving patients, parents/relatives in the planning and delivery of care and development of services leading to service improvement. Contribute to the development of leaflets and information regarding medication and ADHD for children, families, and professionals. Ensure the effective and efficient use of physical and financial resources. Collect, collate, evaluate, and report information and maintaining accurate patient records. Ensure the service is monitored on a regular basis and data is available as a record to provide statistical information to Clinical Leads and others and to provide data to relevant local and national bodies to audit service as appropriate.
