The role includes; but is not limited to the following roles and responsibilities: Working within a particular pathway within the team processing referrals for individuals in settings including Acute services, community settings, Open Rehabilitation or the independent sector (including NHS England commissioned provision). Identifying suitable placements for service users referred into the Placement Service and placement matching services to individuals care and support needs. Referrals to include thorough history and rationale for proposal of placement matching ensuring Service Users rights are upheld throughout the process. To ensure that placements take place in accordance with service user need, financial accountability and governance arrangements. To be a key support and interface with private health and social care providers on behalf of the trust. To ensure that service users are in placements that meet their needs undertaking regular placement reviews in conjunction with care teams to ensure individual's care packages are effective in meeting individual needs. Providing advice and support to Care Co-ordinators, community team representatives and Managers on placement related issues. To work more intensely with Service Users who are identified as being ready or near ready to move/step down from their current placement following review. Updating PARIS regarding any progress notes; light touch or full annual Connected Lives placement reviews. Contributing to intensive time limited pieces of work e.g. relocating all Service Users from a particular establishment for example, as a result of quality concerns. Reviewing placement suitability and Service User mix within placements, and providing a professional perspective of overall modelling of services upon service user changes; and reporting these views into the Placement Contracts, Commissioning & Quality department. Ensuring individuals and their families legal rights are upheld within placement settings- including the consideration of advocacy where appropriate. Attending safeguarding meetings for HPFT commissioned services or service users; undertaking appropriate necessary actions relating to the Placement Team; as well as feeding back to the wider team; and ensuring this is linked to the Contracts, Commissioning & Quality department where required. Undertaking responsive quality review visits upon concerns being raised, to ensure the safety and wellbeing of individuals in placement settings commissioned by the placement service. Supporting the Contracts, Commissioning, & Quality department with any updates and follow up review of action plans relating to placements within their workstream, specific to quality assurance and where required, undertaking a joint visit to placements with such concerns with monitoring officers/ Business and Contracts Manager. Undertaking NHS England mandated quality visits to independent or external NHS trusts in relation to individuals in hospital settings commissioned by the placement team; for those with ASD and/or LD diagnosis. To work in cooperation with care co-ordinators and other agencies in the pursuance of these responsibilities on behalf of the Trust.
