Key Responsibilities To work with the discharge sisters and discharge coordinator to promote the correct implementation of discharge processes throughout Mid Yorkshire hospitals to ensure safe and timely discharge and smooth transfer of patients. Actively participate in discharge planning and feel confident in ensuring that discharge activity is occurring for every patient with No Reason to Reside. Actively work with the discharge coordinators and ITOC teams to ensure patients are identified for transfer to the discharge lounges to ensure acute beds are available as soon as possible. Provide concise and detailed information regarding patients discharge plans. Work closely with all internal and external partners and agencies to ensure a seamless service, including working to reduce delayed discharges. To promote best practice in achieving high quality discharge planning and support in reducing No Reason to Reside numbers. To use tact and diplomacy where there is dispute over complex issues regarding leaving hospital and there may be disputes regarding courses of action to be taken. To liaise with relevant staff and agencies on a daily basis to resolve daily operational issues and ensure discharges are managed effectively. Acts as an advocate for patients and their families/carers recognising the boundaries of their clinical knowledge; liaising and referring on to other services / agencies as required. To understand and apply the skills and knowledge around complex needs and discharge planning. To apply knowledge of the effect of disability and provide advice on discharge planning processes, options/ services available to facilitate discharge and the roles and requirements of the multidisciplinary teams. To progress chase each patient through their hospital stay ensuring diagnostics and results are delivered and reviewed in a timely manner. To use knowledge, training and experience to analyse, plan, and implement discharge plans for patients with complex physical, mental health and social needs. Using knowledge and understanding to support staff from across agencies in planning effective discharges. To be able to use problem solving skills, to determine appropriate actions and formulate specific and appropriate management plans. To have a working knowledge of current legislation, National Service Frameworks, clinical standards and national policies in order to provide an effective discharge service.
