Service Facilitator
Scale 4 + 2 increments for evening and weekend working (£26,409 - £28,163 pro rata)
35 hours per week. There is a six week rota, comprising of a mixture of early shifts (07:00 -14:30), mid-shifts (11:30 19:00) and late shifts (14:30- 22:00). Please note that you will be required to work two weekends out of six.

Your Role
It is an exciting time to join the REaCH service, we are passionate about delivering high quality and person-centred care and support to adults aged 18 and over living in Stockport. The Service Facilitator team are very much at the heart of the service and are an essential part of our ‘back office’ functions. The team are responsible for finding capacity for care visits, setting up and making changes to packages of care and ensuring that people receive a positive and seamless service. The team also support many of our administrative functions including speaking to people who are receiving our support and their carers and relatives, and internal and external colleagues and services.
About You
We are looking for people who are committed to offering excellent customer service, although you don’t need to have worked in the care sector before. You will need to be able to demonstrate excellent communication skills, whether that be face to face, on the phone or by e-mail. You will be confident in the use of IT systems and have the ability to problem solve and work as a team. You will be flexible in your approach and be able to work shifts across a 7-day working week, including on bank holidays.

REaCH is a frontline care service consisting of three main areas; Reablement, Night Support and our End-of-Life Service, this support is delivered by our teams of Home Support Workers working across the borough.
