We are looking for a Community Support Services Coordinator on a fixed term basis to cover for Maternity Leave.
The Brightways Hub is a day service that offers new opportunities both therapeutic and developing individuals’ life skills, to adults and young people within our local community. We aim to offer choice and control to people we support over the service they receive, offering tailor-made packages of support with flexibility around their needs. The day service is based at Oakham Enterprise Park.

About you…
* Qualified to NVQ Level 3
* Experienced in leading and supporting a team operationally.
* Have a positive attitude to disability and an understanding of the purpose of advocacy
* Experience of Person Centred Planning and Health Action Plans
* Good verbal and written communication skills
* Able to prioritise own and others work, and motivate staff members to be the best they can be
* Able to work flexibly as part of a rota to include weekends, bank holidays and evenings if required.
About the role…
* To coordinate and facilitate service provision supporting people who have been assessed as having a need for support, promoting their independence and enabling choice and control.
* To promote social inclusion, reducing isolation and contribute to service development to meet both individuals and service outcomes.
* To maintain the staff rota to ensure adequate support is provided, coordinating annual leave, training, and sickness cover amongst the team.
* Arrange, coordinate and contribute to multi-disciplinary planning meetings for individuals to ensure an integrated coordinated health, social care and community approach to service delivery.
The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.
The Community Support Services Coordinator interviews will be held at Oakham Enterprise Park throughout November. The exact date will be confirmed.
