Job summary A vacancy has now arisen for a dedicated and recovery focused Occupational Therapist to join our service. Oak House provides rehabilitation and recovery services for adults with complex and enduring mental health difficulties. The clinical team at Oak House is small but effective, with a strong emphasis on collaborative working models and strong activity- based interventions. Being part of our experienced team would allow you the opportunity to develop a broad range of skills to support patients to develop their skills and confidence in activities of daily living and accessing community activities. Main duties of the job Daily Clinical handover from nursing team Assessment of patients Activities of Daily Living using the Model of Human Occupation Feeding back to the MDT and community colleagues and partners services on ADL assessments and advising the team on patients skills and needs Planning and delivery of patient therapeutic activities on site and in the community, individual and group Linking in with organisations that can support patients in voluntary or paid work opportunities Supporting patients and their families in discharge planning such as home visits Care planning, risk assessments, discharge planning, MDT meetings Working within the framework of the mental health act Supervision of junior colleagues and student OTs About us At Herefordshire and Worcestershire Health and Care NHS Trust, we are working together to deliver outstanding care.
