The aim of the health and justice service is to focus on reducing health inequalities for vulnerable people who enter into the criminal justice system.
The outreach practitioner role is to work in an outreach service within the wider health and justice service. The outreach service will be a new service that is being developed to support vulnerable people who have entered into the criminal justice system. The service will focus on supporting adults who have been released from police custody and may have additional health needs. The focus of the team is to provide support for a period of up to 12 weeks in the community to reduce risks around reoffending and also support health vulnerabilities. The main focus of the outreach service is to support vulnerable adults to engage in the services available in the community. This may mean supporting people to health appointments, engagement in mental health and drug and alcohol services, and providing psycho-social approaches, advice, and guidance to help support health inequalities and reduce the risks of reoffending.
Main duties of the job
The outreach practitioner will support with triage/referrals, assessment, and support for the adults who are referred to the outreach service. The referrals into the team will come from police custody, liaison and diversion practitioners, and the courts. As a qualified staff member, the outreach practitioner will offer supervision and clinical oversight for junior staff.

Essential
* Registered Mental Health Nurse / Learning Disability Nurse / General Nurse on NMC Register.
* Registered Social Worker.

* Leadership qualification.

* Evidence of extensive post-registration experience with Clients experiencing mental health-related difficulties.
* Experience of working with Clients who have difficulty in engaging with services and have complex mental health needs.
* Experience of working in community settings and making decisions autonomously.
* Experience of assessing, planning, coordinating, and managing people with mental health problems in a community setting.
Skills, Knowledge & Abilities
Essential
