Zennor ward is a 28 bedded eldercare ward that looks after patients with often complex needs. The post holder will:
1. Co-ordinate all stages of the patient journey, from admission to discharge.
2. Proactively support and facilitate the work of the multi-disciplinary team in delivering the best outcome for the patient; informing and communicating full outcomes of assessments and discharge plans within the clinical area relating to patient discharge/ flow.
3. Support the Service Manager/Clinical matron with implementing changes to improve patient experience and discharge experience.
4. The role is an advocate for promoting discharge and works closely with the multidisciplinary teams, including Trust colleagues, Clinical Site Management, Flow Hub, Social Care Providers, our Patients and their families, to proactively plan, prepare and support discharge activity.
5. Ensure electronic systems are updated in real time supporting the overall move away from paper records.
6. Will be a member of the Clinical Site team but work to a specific ward or wards, occasionally may need to work in alternative areas and travel between sites to provide cross cover.
Main duties of the job
1. To have made an effective contribution to reaching the Trust's vision, strategic objectives and key work programmes.
2. Promote early discharge planning and planning of discharge pathways.
3. To be responsible for supporting the discharge planning from the point of admission ensuring that discharges are expedited in a timely manner maximizing the use of hospital beds and reducing length of stay by being a key member of the ward Multi-Disciplinary Team.
4. To liaise daily with medical staff & ward Managers/Staff to review all patients, supporting and facilitating discharge decisions and progress and chasing any outstanding diagnostics and reviews.

Person Specification
Qualifications
* NVQ Level 3 in Care/Business Administration or equivalent qualification or relevant practical experience
* Relevant IT skills to include email, excel, word, and ECDL or equivalent
* Evidence of understanding and insight into current Trust issues
* Some knowledge of infection control
* Knowledge of health and safety issues
* Awareness of legislation relevant to post
Experience
* Worked within a Health or Social Care Setting
* Multi-disciplinary working and ability to participate in interdisciplinary decision making
* Working with PAS, MAXIMS, and Swiftplus
* Experience of ward based work
* Experience of the discharging of patients to further health care settings
