Job summary The vision for an Integrated Discharge Team in Bolton is for the three main stakeholders, the Trust, the Council and the ICB to work in partnership to provide a fully integrated and co-ordinated hospital discharge service for patients admitted to wards in Bolton NHS Trust. The post holder will play a key part in facilitating timely and safe discharges for patients, and liaising with relevant social and NHS staff to support this.
