About the Role:
The Home Health Registered Nurse Supervisor/Case Manager plays a critical role in ensuring the delivery of high-quality healthcare services to patients in their homes. This position involves overseeing nursing staff, coordinating patient care plans, and ensuring compliance with healthcare regulations and standards. The supervisor will work closely with patients, families, and interdisciplinary teams to assess needs and implement effective care strategies. By fostering a supportive environment, the supervisor will enhance patient outcomes and satisfaction. Ultimately, this role is pivotal in bridging the gap between hospital care and home health services, ensuring continuity of care for patients.
Minimum Qualifications:
* Current and valid Registered Nurse (RN) license in the state of practice.
* Bachelor's degree in Nursing (BSN) or higher.
* A minimum of 3 years of nursing experience, with at least 1 year in a supervisory or management role.
* Proven experience in nursing administration, preferably in home care, nursing home, and hospice setting.
* Strong knowledge of Medicare regulations and compliance standards.
* Excellent leadership skills with the ability to manage diverse teams effectively.
* Proficient in developing systems that improve patient care delivery and outcomes.
* Exceptional communication skills, both verbal and written.
* Ability to analyze data and make informed decisions based on patient needs.
Duties:
* Lead and manage the nursing department to ensure the delivery of high-quality care to patients.
* Develop, implement, and evaluate individualized care plans in collaboration with healthcare teams.
* Ensure compliance with Medicare regulations and standards of practice within the agency.
* Oversee the day-to-day operations and field work when needed.
* Provide guidance and support to nursing staff in their daily operations and professional development.
* Monitor patient outcomes and implement quality improvement initiatives as necessary.
* Collaborate with other departments to enhance overall patient care services.
* Maintain effective communication with patients, families, and interdisciplinary teams regarding care plans and treatment options.
Responsibilities:
* Supervise and mentor nursing staff, providing guidance and support in patient care delivery.
* Develop, implement, and evaluate individualized patient care plans in collaboration with healthcare teams.
* Conduct regular assessments of patient health status and adjust care plans as necessary.
* Ensure compliance with state and federal regulations, as well as organizational policies and procedures.
* Facilitate communication between patients, families, and healthcare providers to ensure coordinated care.
