Position: Frailty and Long-Term Conditions Practitioner
Hours: Part time/Full time: Up to 37.5 hours per week
Opportunity: To work across all our sites engaging with a wide range of different multi-agency professionals to proactively support people with frailty and Long-Term Conditions.
We currently have an exciting opportunity for an enthusiastic and forward-thinking Registered Nurse who is looking to enhance and develop their career in the primary care setting. We need you to help us develop a proactive approach to population health management to identify people who require quality care to manage the impact of their condition. We are looking at the offer of a structured holistic assessment to those identified to be most at risk.
You will be working as an autonomous practitioner as well as collaboratively with the wider integrated frailty service offered by our community colleagues. You will work as part of our Frailty Service within our Wellbeing Team and other practice staff to make this a key role in delivering care to this patient cohort.
Main Duties of the Job
1. To build and continually advance own clinical knowledge, skill and competence based on current evidence through educational programmes.
2. Provide specialist advice around the management of fragility to others.
3. To work in collaboration with other professionals to deliver and ensure care is delivered appropriately.
4. To demonstrate clinical effectiveness of service delivery by use of evidence-based practice and outcome measures.
5. To have the skills to take a history, assess, examine, diagnose, and develop a management plan.
6. To recognise, assess and manage risk across the immediate and wider working environment and make appropriate decisions autonomously ensuring statutory requirements are met.
7. To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
8. To communicate effectively verbally and in written form in the exchange of highly complex, sensitive, or contentious information in demanding situations using de-escalation, mediation, resolution, and professional Duty of Candour.
9. To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints.

Clinical Responsibilities
1. To assess, plan, implement and evaluate care plans for patients; promoting proactive independence and autonomy; working within a multi-disciplinary team.

Person Specification
Qualifications
* Registered practitioner with further professional qualifications or demonstrable extensive experience in the relevant specialty.
* Membership of the relevant Professional Body.
* Evidence of recent professional development in an up-to-date portfolio.
* Full Driving licence.
* History taking and Physical Examination course HAPE or Enhanced Assessment Skills for Clinical Practice.
* Knowledge and understanding of primary care relevant to role.
Experience
* Experience at Practitioner Band 5 level or above.
* Experience of managing change.
* Demonstrable knowledge of assessment and therapeutic interventions in area of frailty.
* Able to demonstrate specialist clinical reasoning skills to assimilate information to make a clinical judgement regarding diagnosis and intervention.
* Ability to prioritise and organise workload effectively.
* Experience of working with Frailty and Long-Term Conditions.
* Recent previous experience within a comparable role.
Knowledge, Skills, IT & Other Experience
* Evidence of demonstrating the organisations values and behaviours.
* Able to communicate effectively at all levels of an organisation and with staff, patient/service users, visitors, or external organisations both verbally and in writing in the exchange of highly complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills.
* Able to analyse and assess situations and to interpret potentially conflicting situations and determine appropriate action.
* Experience of planning and organising complex activities.
* Ability to use own initiative within sphere of authority.
* Knowledge and understanding of legislation relevant to practice.
* Demonstrable leadership qualities and the ability to perform as a role model.
* Willingness to advance own clinical knowledge, skill and competence based on current evidence.
* Able to travel using own vehicle.
* Hold a Full UK Driving Licence.

Salary: £28,254.99 to £40,925.74 a year Pro Rata Depending on Experience
