General Practitioner Lead supporting the Care Home Team
Following the release of ARRS GP information and funding, we are looking for a newly qualified (within 2 years and not having previously held a substantive GP post), enthusiastic, forward-thinking GP to join Pontefract & Knottingley PCN as GP lead for our Care Home Team and practice work. This is a fixed term salaried contract until 31 March 2025; however, this may be extended subject to new contract.
We are flexible with the days/times of work, and these could be within school hours only if desired.
Main duties of the job
As GP lead for our Care Home Team, you would provide clinical support to our established team who provide acute and routine care to the Care Homes in our PCN. This team consists of:
* 2 ANPs
* 1 Nurse
Leadership and supervision will be provided by the PCN Education Lead, as well as a named mentor (you will have opportunities to meet regularly for debriefs). Tutorials and personal development time will be factored into your working week.

Pontefract & Knottingley PCN has a total population of over 49,000 patients across 4 GP practices, all rated 'Good' by CQC. This is a rare opportunity to gain experience across four different practices and support our Care Home Team. Our PCN staff currently includes a well-established Pharmacist Team, an Occupational Therapist, Health Coaches, Social Prescribers, Care Coordinators, GPAs, a Mental Health Practitioner, and ANPs attached to our Care Home Team.
We pride ourselves on having a team of excellent educators, with two registered training practices, and a third practice currently working towards this with the support of our PCN Education Lead GP. We can offer a comprehensive training and education package to develop skills in your chosen field. By joining the PCN, you can learn how to support patients with integrated care across other health and social care providers, as well as collaborate with practice and PCN multidisciplinary colleagues. This would be a fantastic opportunity for you to learn from experienced GPs and take part in case reviews and MDT meetings further expanding your knowledge of Primary Care while also being part of a forward thinking PCN.

Knowledge and Understanding
* Understanding the role of medical services in a wider public health agenda.
* Knowledge of the demographic characteristics that affect health and health care in the area.
* Understanding current NHS policies.
Other
* Committed to working as part of a multi-disciplinary, multi-agency primary care team.
* Openness to explore alternative working practices.
Experience
* Must not have been substantively employed as a GP in general practice previously.
* Not employed in a temporary capacity as a locum.
* Upon commencement of employment, must be less than 2 years beyond their certificate of completion of training issued by the GMC.
* Experience of working in primary care in an area with significant health problems.
* Experience of providing primary care in and out of hours.
* Development of areas of clinical expertise.
* Experience of audit and improving care for patients.
Qualifications
* MBBS or equivalent medical degree.
* Full registration with GMC.
* Certification of Completion of Training (JCPTGP PMETB).
* Inclusion on Performers List.
* MRCGP.
* Evidence of a commitment to ongoing personal & professional development.
Personal Skills and Qualities
* Ability to communicate clearly and effectively in English, both written and verbally.
* Good organisational skills.
