Our service is expanding, and we are offering an exciting opportunity to join our busy Integrated Care Coordination for Urgent Care Team as a Band 7 HCPC / NMC Registered Clinical Team Lead based in Hanley, Stoke-on-Trent. The service is developing a multidisciplinary approach and would welcome applications from AHPs to support this.
We are looking for a passionate and dedicated Clinical Team Leader to provide management, support, clinical escalation, and professional expertise to the team of Clinical Triage staff. The team provide telephone support to health and social care registered professionals requiring assistance with Admission Avoidance. This ranges from signposting the caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment across Stoke on Trent and Staffordshire. We aim to provide the right care, in the right place, at the right manager; you will promote seamless patient care, striving to act as an ambassador for good quality care and sharing and celebrating team achievements.
You will need to be flexible, approachable and have a sound clinical background and knowledge. You will need to be friendly but assertive, and be confident in your own decision making, being able to challenge when necessary.
You will be required to cover the 7 day period, 365 days a year and therefore weekend and out of hours working is required.
Main duties of the job
You will work as a key member of the Integrated Care Coordination Team to ensure professionals are able to make referrals through one access point. You will be leading a team of Clinical Triage staff, supporting them to work independently and to exercise their assessment and evaluation skills to their full ability in order to avoid inappropriate hospital admissions. You will support the team to adjust care programmes as necessary, and instigate changes that are evidence based and in the patient's best interests. You will be first line of clinical escalation for the team.
As part of this role you will also be expected to:
Have oversight of the shift and ensure the team provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement.
Ensure that polices and processes are followed to keep both staff and patients safe
Provide immediate advice and direction to team members in relation to operational service issues, analysing and interpreting the various options available
Ensure quality is embedded within all aspects of service delivery and owned by staff.
Support staff to actively engage in developing quality improvement projects within their team.
Create a team culture in which staff are valued and well-supported and able to deliver services with care and compassion.
