An exciting opportunity has arisen for a full-time Senior Care Act assessor within a well-established and dynamic initial response team.
The position is within the older persons with physical disability team, which is a community-based team working with Adults over 18 years of age with a range of physical and mental health needs. The successful applicant will hold a Level 3 Diploma in Adult Care or Lead Adult Care/Social Care Worker, and/or Level 4 Diploma in Adult Care or Lead Practitioners in Adult Care Level or be willing to complete/achieve this if a Level 3 qualification is already held. The successful candidate will also have experience working within a relevant field.
This is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues in the promotion of independence and improving the health and wellbeing of the population served.
Main duties of the job
The post holder will be expected to access social care and health records to gather information; therefore, it is important for the post holder to be able to use a computer and have an understanding of the importance of good recording. You will require effective communication and written skills and be willing to work as part of a team to promote the best outcomes for individuals. You will be required to be self-motivated and organized in order to manage any competing demands and be provided with all relevant training and support in the role.

You will also receive the support of your team and your line manager, who will provide regular supervision both on a formal and informal basis.

Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e., stage 1 (ASYE), 2 (Social Worker), or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

* Experience of working in an NHS/Local Authority setting where MDT and specialized practice associated with social work and social care is required
