Job summary The position will be for the successful applicant to work within the Carrick iCMHT based in Truro and covering the Carrick district including surrounding areas, St Agnes and Falmouth. The successful applicant will be expected to work in the Community with individuals with Severe and Enduring Mental Health Conditions. It is essential you have good communication skills. You will be part of a multi-disciplinary team, contributing to the effective provision of Mental Health Services to an identified caseload of service users. You will provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans provided by their lead health care professionals. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Main duties of the job Some of your role will include the need to: Foster and maintain good working relations with the integrated Community Mental Health Teams and other services. Work within the Clinical Support Team across the Mid Cornwall Locality Establish and maintain effect contact with service users, their relatives and carers in order to ensure good communication. Give information and assistance to service users and their carers so that they are aware of their rights, entitlements from the team and wider health and social care community. Use of appropriate community resources appropriate to the needs of the service user. Practice in a way that actively minimizes dependency and promotes recovery. Work with service users and their carers to access identified services. Monitor mental health and be aware of fluctuations in mental health through observations and report changes to the team practitioners so that service users are directed to appropriate services and risk is managed.
