Are you passionate about the delivery of high quality palliative care to people living and dying with dementia?
Due to maternity leave we require an enthusiastic and passionate nurse in palliative/dementia care (RMN/RGN/RNLD), to fulfil a nursing role within the Dementia Palliative Care Team for a period of 12 months.
You will work as an integral part of a small team supporting people with dementia and palliative care needs (and their carers) across Derbyshire.
The primary goal of the team is to enable improved palliative care for people living with advanced dementia, with mental and physical health needs in care homes/supported living or their own home.
The team is focussed on ensuring the person remains at home (wherever home is) and avoiding unnecessary acute care admission.
The team will also support discharges from acute hospital beds and pathway 3.
The post holder will work as an integral part of a small team supporting people with dementia and palliative care needs (and their carers) across Derbyshire.
The post holder will work towards the primary goal of the team is to enable improved palliative care for people living with advanced dementia, with mental and physical health needs in care homes/supported living or their own home.
The post holder will provide specialist support to both patients and their carers who experience or are at risk of experiencing difficulties in the last 6–12 months of life due to dementia.

This team has developed as a collaborative resource in response to a gap in the current pathway of care for people living with advanced dementia with palliative care needs.
Training and supervision will be provided to enable individuals to develop their skill set, and integrated working within the team will support case management.
The post holder will:
• carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support through holistic dementia palliative assessment and person-centred care planning.
• primarily cover the Chesterfield area.
• have a current full driving licence as role will require travel across Derbyshire.
• provide specialist support to both patients living and dying with dementia and their carers with identified palliative care needs.
• support GPs and primary care teams with complex care planning.
• be focussed on ensuring the person remains at home (wherever home is) and avoiding unnecessary acute care admission.
• support discharges from acute hospital beds and pathway 3 beds.
• carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support.
• liaise direct with colleagues from the Older Adults Community Mental Health Teams and the Dementia Rapid Response Team.
