Job overview
Within this job share role, the post holder will be clinically responsible for the assessment of patients, development of care plans, and the implementation of care.
He/she will provide advice and guidance to the multidisciplinary team and ensure all records and documentation are completed and relevant.
This role involves overall 24-hour responsibility for nursing care within a designated neighbourhood and requires enhanced communication skills to deal with the broad range of issues involved in the day-to-day leadership of the integrated nursing team.
The post holder is responsible for the day-to-day operational management of the nursing team within the neighbourhood, providing assurance to the Community Matron.
The role includes managing staff, coordination of care, CPD, safeguarding, HR and performance issues, staffing lists, incident management; ensuring teams deliver a safe high-quality service, appraisals, recruitment and selection, resource control, and budgetary management for the nursing team.
Main duties of the job
The post holder will be a core member of the neighbourhood management team, working collaboratively with other neighbourhoods for health and social care services to deliver high-quality care and develop a locally appropriate integrated approach to care delivery.
The post holder will contribute to the improvement strategy and represent community nursing at forums and groups across the Neighbourhood and Locality.
