The post holder will carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support through holistic dementia palliative assessment and person centred care planning. This team has developed as a collaborative resource in response to a gap in the current pathway of care for people living with advanced dementia with palliative care needs. Training and supervision will be provided to enable individuals to develop their skill set, and integrated working within the team will support case management. This role will be covering the South of Derbyshire and site of base will be Florence Nightingale Community Hospice in Derby city. You must have a current full driving licence as role will require travel across Derbyshire. The post holder will provide specialist support to both patients living and dying with dementia and their carers with identified palliative care needs. The post holder will support GPs and primary care teams with complex care planning. The post holder will be focussed on ensuring the person remains at home (wherever home is) and avoiding unnecessary acute care admission. The post holder will also support discharges from acute hospital beds and pathway 3 beds. The post holder will carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support. The post holder will liaise direct with colleagues from the Older Adults Community Mental Health Teams and the Dementia Rapid Response Team.
