Suitably experienced post-holders will be expected to provide a clinical service in the relevant specialty to patients of the Worcestershire Acute Hospitals NHS Trust and to General Practitioners. They will attend multi-disciplinary meetings and work within multi-disciplinary teams to provide high-quality clinical care. It is anticipated that work will be undertaken on three sites and closer to the patients' homes.
The Trust is seeking to extend services to GP surgeries and health centres. Job plans may be revised in due course. Post-holders will provide support to the A&E department and acute medicine to ensure timely and appropriate care and patient flow through the hospital. Responsibilities include:
1. Providing consultation and advisory services to clinical colleagues in the Trust and Primary Care.
2. Offering clinical opinion and support for teams in community hospitals and neighbourhood teams.
3. Supporting GPs in managing patients in their homes and care homes.
4. Conducting clinical reviews of high users of community and secondary care.
5. Providing face-to-face, phone, and email advice using electronic patient records.
6. Mentoring generalist clinical staff and supporting enhanced training.
7. Encouraging a Comprehensive Geriatric Assessment approach for older patients.
8. Promoting anticipatory planning for managing acute deteriorations.
9. Supporting management and transfer decisions in all care settings.
10. Educating those involved in managing older people living with frailty.
Post-holders will attend cross-county and cross-site meetings to collaborate with colleagues in delivering a high-quality, timely service and to meet the Trust's and departmental objectives. They may participate in the on-call rota to ensure emergency and urgent services are provided out of normal hours, shared equally with consultant colleagues. The Trust expects consultants to attend the hospital site to review patients when clinically necessary during on-call duties.
The consultant on call must be prepared to attend whenever requested by a member of the medical and nursing team. Post-holders will develop close collaborative links with clinicians in their area of expertise and work to develop services to the highest standards, according to departmental protocols and Trust guidance. This includes developing diagnostic and therapeutic protocols and guidelines, with active participation from Primary Care colleagues.
Post-holders will participate in the Trust's annual appraisal process as described by DOH, leading to revalidation and job planning. They will seek approval for any absence from the Trust, such as annual leave, professional leave, and study leave, and make this record available at their job plan review.
