An exciting opportunity has arisen within the Mid Ross District Integrated team for a Band 4 Health and Social Care Coordinator.
The HSCC plays an integral role within the Integrated Team gathering referral details from various sources including clients, their family members and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team in order to prioritise and allocate. The HSCC provides support to professional staff when dealing with urgent and critical cases in the community and also holds a non-complex case load such as telecare assessments and clients referred through the right call for a fall pathway. The HSCC plays a key role in supporting patient flow both from acute and community hospitals. This involves close working with colleagues in the Mid Ross Integrated Team and the Acute Hospital Team.
The successful candidate must have excellent communication skills, good time management and organisational skills and have a flexible approach to their work. A good understanding of IT systems and databases is also essential.
Full driving license and use of car for work purposes is essential.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
