An exciting opportunity has arisen within the Gloucestershire Continuing Healthcare (CHC) Team for a Band 7 Workstream Lead. The role is planned to build on a combination of CHC subject matter expertise, and skills to develop and support a strong service delivery and work within a great team.
Main Duties of the Job
The post holder will play a key role across the CHC service, working within the remit of the National Framework for Continuing Healthcare, to deliver an effective, efficient, and equitable service to the people and populations of Gloucestershire.
The post holder will be an expert in NHS CHC and associated policies and will be able to communicate clearly and knowledgeably. The post holder will also contribute their clinical expertise to the CHC process and associated pathways/workstreams. You will lead and manage a team of nurses to deliver multi-disciplinary CHC assessments for patients in different settings/environments in line with standard operating procedures and policies.
As a Team Lead, your role will include the delivery and oversight of a variety of functions across the CHC service, including Assessment, Reviews, Care Planning, and Case Management. The post holder will have experience of working within health and social care and have excellent verbal and communication skills. You will also be required to work closely with patients, families, providers, and other external partners.
As a Team Lead, you will also have the opportunity to work across other areas of CHC, including audit, appeals, verifications, complaints, and more. You will be supported to further develop your skills and experience to work across the whole pathway, safely, confidently, and effectively.

Person Specification
Qualifications
* Educated to degree level in a relevant subject or equivalent level qualification or significant experience of working at a similar level in a specialist area.
* Registered Nurse Qualification.
* Post-graduate degree in Management Studies or equivalent.
* Significant experience of successfully operating in a politically sensitive environment.
* Evidence of continued professional development.
* Demonstrated experience of coordinating projects in complex and challenging environments.
* Experience of managing risks and reporting.
* Experience of drafting briefing papers and correspondence at SMT level.
* Experience of monitoring budgets and business planning processes.
* Understanding of the public sector.
* Demonstrated experience in a healthcare environment.
* Comprehensive knowledge of project principles, techniques, and tools, such as Prince 2 Foundation and Microsoft Project.
