Discharge to Assess Social Worker
We are looking to recruit a Qualified Social Worker for the Discharge to Assess team with the Social Care industry in Bristol City Council. This vacancy will be for an initial 3-6 months with the potential for extension subject to internal circumstances and your performance.
We are seeking dedicated and compassionate Social Workers to join the discharge teams at Southmead and Bristol Royal Infirmary Hospitals, employed by Bristol City Council.
You will have a passion for enabling service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support.
Responsibilities include:
1. Completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation.
2. Developing comprehensive discharge plans.
3. Collaborating with medical staff to ensure seamless patient care transitions.
Minimum Requirements:
1. HCPC Registration
2. 1 year post qualifying experience within the UK Social Care industry
3. Enhanced DBS/CRB (we can obtain for you)
4. Eligibility to work in the UK
