Job summary We are delighted to advertise an exciting opportunities for 2 Advanced Clinical Practitioners to join our General Surgery Department at Epsom and St Helier. You will need to be qualified and practising autonomously as an advanced clinical practitioner. As these are new roles in the Trust and Division, you will be pivotal in development of this post. The post holder will demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families, and carers. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice in both an emergency and elective setting. Main duties of the job Undertake both elective outpatient clinics as well as supporting the emergency surgery service. Assessing individuals, families and carers, holistically using a range of different methods, such as physical assessment and history taking, ordering and interpreting diagnostic tests or advanced health needs assessments. Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnosesCommunicate effectively with patients, carers and the multidisciplinary teamDecide on and carry out treatment, which may include the prescribing ofmedicines, or referring patients to an appropriate specialist.Ensure the provision of continuity of care. Assess and evaluate with patients and / or carers, the effectiveness of thetreatment and care provided and make changes as needed.
