JOB PURPOSE: Provide dietetic support for patients with Type 1 and Type 2 Diabetes seen in the Richmond Community Diabetes Service. To be responsible for the development and provision of a comprehensive and effective, evidence-based dietetic service for Cardiac Rehabilitation patients referred across Richmond. Provide nutrition support for patients on the inpatient unit at Teddington Memorial Hospital. To deliver clinically excellent, person-centred care that promotes self-management and well-being.
KEY RESPONSIBILITIES:
1. Work as a member of the Diabetes Team, providing a comprehensive diabetes dietetic service for patients with Type 1 and Type 2 Diabetes.
2. Provide dietetic support for patients with cardiac conditions known to the cardiac rehab service.
3. Undertake comprehensive specialist assessments, set patient-centred goals, create treatment plans, monitor and evaluate a designated caseload of patients, using evidence-based clinical pathways.
4. Formulate treatment plans and goals in partnership with the patient, clearly communicate your clinical reasoning to justify your recommendations utilising the most appropriate treatment/intervention.
5. Provide specialist clinical advice, support and educate patients and carers in the management of their condition.
6. Use counselling and motivational interviewing skills to enable, motivate and empower people with health conditions to make sustainable lifestyle changes.
7. Liaise effectively with all members of the multidisciplinary team/departments/services/external agencies to ensure the delivery of an effective and coordinated approach to service delivery.
8. Plan, deliver and review treatment programmes by providing timely information and advice to GPs, AHPs and Practice Nurses regarding the treatment plan and dietetic recommendations.
9. Ensure safe practice through the development of a caseload management plan taking into account risk management and the documentation and reporting of critical incidents as outlined in trust policy.
10. Attend and participate in team meetings.
COMMUNICATION:
1. Communicate and support the patient in understanding and managing aspects of their condition, which may at times be distressing, sensitive and unwelcome.
2. Gather information from patients, relatives and other services (from a variety of sources including electronic patient records) and maintain clinical documentation in line with the Code of Conduct.
3. Advise and educate patients and carers in a variety of ways which may include suggesting techniques and strategies to manage their condition.
4. Communicate complex or sensitive information, in an empathetic and reassuring manner to patients and carers, including to those whom English is a second language.
5. Collate feedback and information from patients/carers and their families to inform service evaluation and associated service improvements.
6. Be able to communicate appropriately over the telephone, email and Teams.
