Job Description: Role Overview: As a Homeless Support Worker for this organisation, you will play a crucial role in supporting homeless individuals through both office-based and outreach activities. You will work closely with clients to assess their needs, provide essential services, and connect them with resources to improve their living conditions and overall well-being. Key Responsibilities: Client Assessment and Support: Conduct comprehensive assessments of clients needs and develop personalized support plans. Provide emotional and practical support to clients, helping them navigate housing, healthcare, and social services. Maintain accurate and up-to-date records of client interactions and progress. Outreach Activities: Engage with homeless individuals in the community through outreach programs.
