Are you an enthusiastic, registered Mental Health Nurse or Social Worker looking to work across teams within a friendly service? The Windsor, Ascot and Maidenhead (WAM) Older Peoples Mental Health Service is looking for a motivated Mental Health Practitioner to join our friendly and innovative team.
You will have the opportunity to work across CMHT(e), HTT and Memory Clinic and you will be responsible for managing your own caseload, working collaboratively with other disciplines from both statutory and voluntary sectors.
You may be required to take a lead in an area of special interest, such as working with carers or Younger People with Dementia. You will be responsible for ensuring clinical governance guidelines are followed and to be involved in delivering training and educational programmes as well as participating in audit and research.
The role is based at Nicholson House, Maidenhead and your time in this role will be spent visiting clients in their homes in the local community so you will need a car to get to appointments. You will be required to come to the office occasionally for team meetings, training or for other service-related reasons as necessary, although currently we conduct most team meetings virtually this may change in future.
Main duties of the job
* Participating as a member of a multi-disciplinary team managing a caseload of patients.
* Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
* To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
* Supervising Support Workers and/or students.
* Providing advice to other members of the service and agencies on the needs of service users.
