Full job description
The following are the core responsibilities of the Practice Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. To develop, implement and embed health promotion and well-being programmes.
2. To implement and evaluate individual treatment plans for chronic disease patients.
3. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients' health.
4. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
5. To provide wound care (Doppler etc.) to patients.
6. To provide travel medicine advisory services.
7. To request pathology services as necessary.
8. To process pathology results as required.
9. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
10. To maintain accurate clinical records in conjunction with extant legislation.
11. To ensure read codes are used effectively.
12. To maintain chronic disease registers.
13. To develop, implement and embed well woman clinics.
14. To develop, implement and embed well man clinics.
15. To chaperone patients where necessary.
16. To assist GPs with minor surgery, joint injections and coils when required.
17. To prioritise health issues and intervene appropriately.
18. To support the team in dealing with clinical emergencies.
19. To recognise, assess and refer patients presenting with mental health needs.
20. To implement vaccination programmes for adults and children.
21. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice).
22. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
23. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
24. To support the clinical team with all safeguarding matters in accordance with local and national policies.
25. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
26. To deliver opportunistic health promotion where appropriate.
Clinical skills will include some or all of the following (this list is not exhaustive):
* Wound care, dressings, and general treatment room skills.
* Urinalysis.
* Contraceptive advice, family planning, sexual health.
* Cytology.
* Warfarin (INR) monitoring.
* Maintaining registers.
* Ear syringing.
* ECG.
* BT, BP, height/weight.
* Diet and lifestyle advice.
* Immunisations (childhood, flu, pneumonia, pertussis, shingles, etc.).
* Injections (B12, Triptorelin, Zoladex and Depo etc.).
* Travel vaccinations (only NHS).
* Stock checking and ordering of vaccines.
Chronic Disease Management skills will include some or all of the following (this list is not exhaustive):
* Hypertension.
* Mental health.
* Diabetes.
* Asthma/COPD.
* CHD.
