As part of our medicine management team, you will be responsible for optimising and ensuring the safety of our patients' medication through medication reviews, repeat prescribing, and medicines management following discharge from hospital. You will be involved in annual QOF and IIF targets, and audit requirements. You will be on a rotation with our other pharmacists managing our Telemeds process and working some evening clinics. You would be part of a group of pharmacists and GPs who are keen to develop new services, like for instance a pharmacist-led ADHD clinic.
Main duties of the job
* Playing a key role in our Medicines Management Team to ensure medicines management optimisation and safety. It will include multidisciplinary working with other clinical teams.
* Undertaking clinical medication reviews with patients and producing recommendations for GPs and Nurse/Nurse Practitioners on prescribing and monitoring.
* Having oversight of reducing polypharmacy, which is one of our key outcomes as a green impact practice.
* Reviewing and actioning any necessary changes to repeat prescribing and medicines management policies.
* Managing repeat prescribing re-authorisation process.
* Reconciling medicines following discharge from hospital, intermediate care, and into care homes.
* Undertaking clinical/structured medication reviews.

Person Specification
Qualifications
* Registration with the GPhC
* Degree in Pharmacy
* Non-medical prescriber qualification, or working towards this
Experience
* Experience of planning, management, monitoring, advising, and reviewing general medicine optimisation issues in core areas for long-term conditions
* Experience of medication reviews
* Previous NHS Primary Care Experience
* Able to work under pressure to meet deadlines
* Able to work independently and as part of a team
