An exciting opportunity has arisen for an existing or newly qualified band 5 to obtain a role in the Crisis and Home Treatment Team. We will support the successful candidate through induction and completion of preceptorship.
The Crisis and Home Treatment Team provides crisis assessment and home treatment to patients in acute mental health crisis in the least restrictive environment. CAHTT also works closely with inpatient services to facilitate early planned discharge.
The role includes assessment and home treatment in locations including the patient's home within the Community and/or treatment rooms based at the Stonebow Unit.
Crisis & Home Treatment Team operates a 24-hour service 7 days per week. We are looking for a dynamic and enthusiastic individual who is able to work 37.5 hours per week, over 3/4 days/nights (12-hour shifts).

Main duties of the job
It's a little unpredictable, as you can be called at any time to a crisis situation. We discuss referrals each morning and three times weekly we review our caseload within our MDT.
We complete comprehensive risk assessments for every patient, these can be completed independently although sometimes we discuss more complex patients as a Multi-disciplinary team.
We liaise with families and carers, GPs, community mental health teams, social care teams, community nurses, physiotherapists and voluntary organisations, and anyone who is necessary to maximize recovery.

Job responsibilities
Operate as part of a multidisciplinary team providing Crisis & Home Treatment for people aged 17 and over.
Use initiative and take responsibility for oneself and the quality of work and the service provided to patients.
Proactively engage in formal and informal education for partnership agencies as well as appropriate students.
Actively and creatively engage patients and carers encouraging them to make appropriate use of the mental health and voluntary services available as part of a process of recovery and crisis management.
Undertake full psycho-social assessments with those in need, ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure.

Skills & Abilities
Essential
* Ability to communicate clearly and effectively in English in both verbal and written formats.
* Computer literacy.
* Ability to meet the travel requirements of the role.
Qualifications
Essential
* Registered Mental Nurse.
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Experience
Essential
* Demonstrable post-qualification experience in acute mental health.
* Undertaking multi-disciplinary assessments.
* Undertaking mental state and self-harm assessments.
* Assessing and managing risk.
* Working within the Care Co-ordination framework.
Desirable
* Experience of assessing people who deliberately self-harm and those diagnosed with personality disorder.
* Experience of working with people who engage in substance misuse.
* Experience of working with/assessing people with cognitive impairment and/or functional mental illness.
* Working with user and carer groups.
Knowledge
Essential
* Good working knowledge of Mental Health Act legislation and Mental Capacity Act 2005.
* Clinical Governance, Risk Assessment and Adult/Child Protection Legislation.
