Main duties of the job The Memory Clinic Community Mental Health Support Worker will work as a core member of the Memory assessment services (MAS.) The primary aim of the MAS is to deliver specialist assessment, diagnosis and post-diagnostic support for people referred with suspected dementia and to reach the commitments set out by NHS England. This role is supporting dementia diagnostic services, treatment initiation, titration, and review, post diagnostic care and support, care planning, signposting. The support worker will primarily support with medication titrations initiated within the Memory Assessment Service or Consultants, following the guidelines for Shared Care. This includes working in the community and within a clinic setting. The support worker will work in accordance with an agreed plan of care and under the direction and supervision of professionally qualified staff to promote the maintenance and achievement of recovery focussed goals for the service user including post diagnostic support.
